As Rhian, the All-Wales Regional Manager, I have witnessed firsthand the pressing need for a systematic approach to address these challenges and elevate the quality of care.

Throughout my 29-year professional journey as a paediatric nurse in Cardiff and South Wales specialising in complex care, learning disabilities, mental health, and clinical leadership, I have observed numerous challenges. Families often feel overwhelmed and unsupported due to inadequate respite provisions and a lack of qualified, appropriately trained staff. This situation frequently forces families to consider residential services, separating children from their support networks and often requiring geographical relocation.

Following challenging hospital admissions, sometimes due to a deterioration in their condition, children and their families require additional extended hospital stays while seeking an appropriate care package. These stays expose them to additional risks, such as infection, and increase stress on the family, with other dependents and family members often feeling excluded, causing more pressure and risking relationship breakdowns.

Workforce Pressures

Recruitment challenges are significant, particularly for roles requiring specialisation in mental health and learning disabilities. Ensuring safe and effective practices necessitates ongoing training and the implementation of a consistent rota system to build trust and familiarity between caregivers and families.

Service Gaps

There are noticeable gaps in service continuity and the availability of bespoke care packages, which are crucial for meeting the unique needs of each child and their families. Lack of consistency often leads to excessive unnecessary stress placed on families causing mistrust of services and agencies.

Understanding registration with the Care Inspectorate Wales (CIW) goes beyond mere compliance; it embodies a commitment to maintaining high standards of care, safeguarding, and oversight. Delivering high-quality care to children with complex needs is all about focusing on a few key areas. It is essential to prioritise clinical governance, making sure we have solid systems in place for safe and consistent care delivery. We strive to implement strong risk management and conduct clinical audits to keep our standards high and our service users safe. It’s vital that our staff have the right experience and qualifications to handle complex needs, always keeping a person/family-centred approach in mind.

Regular competency assessments and training is essential to help our staff maintain and enhance their skills. We ensure that our team receives specialised training tailored to the specific physical or mental health needs of the children we provide care for. We also make sure that all staff have the appropriate qualifications and that we regularly assess their competencies to keep everything aligned with local health board standards along with supervisions to support the staff and identify goals, training, and encourage growth of our staff.

We collaborate with various agencies to provide holistic care. We recognise the importance of reaching out to the wider multi-disciplinary team and working together with local health boards, educational institutions, and social care. By doing this, we can ensure a comprehensive approach when supporting families, making sure that the personalised goals we set evolve as the child’s and family’s needs change. We also ensure medication needs, diet/feeding, and other interventions are up to date to reflect any medical changes.
