Our use of shared, real-time care records means families and professionals can access consistent, up-to-date information without relying on multiple phone calls or emails. By having clear, documented communication that is accessible to all, the right people are informed of the right things, at the right time. If escalation is required, our clearly documented records can be shared with GPs, consultants, or other healthcare professionals to help address an individual’s needs.
