Our established nurse-led service cares for generations of people – from babies who require paediatric care at home, children with learning disabilities through to adolescents who have had a life changing accident to adults and older people living with neurological conditions of physical disabilities.
How do we support you independently living in your home?
- Working in collaboration with all other Healthcare Professionals by ensuring we work as a multi-disciplinary team that works to improve individuals’ outcomes.
- Personalised Clinical Care Plan and Electronic Patient Notes system to which loved ones can have access to where consent is given.
- Medications management and administration.
- Clinical oversight on all Care Delivery whether staffed by experienced Nurses or by trained and experienced Care Professionals.
