Post Op & Recovery
Reablement care, also known as intermediate care, is a type of short-term home care to support recovery after a hospital stay, illness or accident. Reablement care is goal-specific – a targeted objective that once achieved allows the reablement care to finish.
Reablement care allows people in hospitals or other medical facilities to be discharged and recover in the comfort of familiar surroundings. Not only does this reduce pressure on the hospitals and the care system in general, but it is also beneficial for the individual. Studies show that the best outcomes are achieved when an individual recovers at home, as opposed to a hospital setting.
Reablement care is:
- Short-term, usually two to six weeks.
- Intensive support, often from a range of professionals.
- Goal specific, agreed by the individual and professionals.
- Focusedon helping the individual practice and become capable of doing things for themselves.
We have been providing reablement care services for several years. Our highly trained and experienced carers work alongside other care and nursing professionals to deliver reablement care in the home.
Our experienced carers regularly visit patients’ homes to help them relearn tasks and practise the daily living skills they need to become independent in their own home. The focus is very much on supporting the individual to do tasks on their own, rather than doing tasks for them.

- Our experienced and trained Carers and Nurses in collaboration with all other Healthcare. Professionals by ensuring we work as a multi-disciplinary team that improves outcomes.
- Personalised Clinical Care Plan and Electronic Patient Notes system which loved ones have access to as well as your wider healthcare team.
- Medications management and administration.
- Clinical oversight on all Care Delivery whether staffed by experienced Nurses or by Trained and Experienced Care Professionals.
